Sensory Profile

This includes a questionnaire (The Sensory Profile 2) and a 60 minute feedback session. Report and resource/referral list will be provided.

Informal Assessment

This is a 90 minute interview with an hour feedback session to review findings. You will receive a confirmation letter for personal use. Referrals and resources will be provided.

Formal Assessment

This is a two hour interview with an hour feedback session to review findings. You will receive a comprehensive report that can be used for some medical and educational accommodations. Referrals and resources will be provided.

Should you get an assessment?

This is a personal choice that only you can make. I can share some reasons people seek assessments and risk involved. 

People often seek assessment to gain work and college accommodations and for personal insight and understanding. Assessments do not typically help with obtaining state disability services. 

If you utilize insurance or superbills your diagnosis will be shared with your insurance provider and will be on your medical record.


 

Method

I provide a robust assessment that includes multiple assessments and most importantly a diagnostic interview that focuses on your lived experience and personal observations. Assessments provided include; ASRS, GAD-7, PHQ-9, ACES, MDQ, Sensory Profile 2, RAADS-R, AQ, CAT-Q, Brown EF/A, and the MIGDAS-2.


 

Ethics

I aim to provide assessments that de-pathologize and center your personal lived experience. I hope to give you insight into yourself and interpersonal relationships. I operate under a lens that is trauma informed, relational, and takes into account the systemic and social issues. 

Historically, assessments and diagnosis have been extremely pathologizing and often focus on white men, leaving a gap in understanding for women, BIPOC folks, and queer people. Often women are diagnosed with Borderline Personality Disorder and BIPOC people are diagnosed with mood disorders, while that is sometimes an accurate diagnosis, I want to ensure that you are seen and heard so that the most accurate diagnosis is provided. 

There is often fear of stigma around a diagnosis or concern for safety. Those are absolutely valid concerns that we can discuss.

I am in network with Aetna, Blue Cross Blue Shield, Cigna, and United Health Care.

Self-Pay

  • Sensory Profile - $400

  • Informal Assessment - $650

  • Formal Assessment- $1000

Sliding Scale and Pro Bono

Oak Therapy also offers sliding scale fee slots $10-$250 (sensory profile), $10- $350 (for the informal assessment), and $10- $600 (for the formal assessment)

Oak Therapy believes in making mental health accessible for all and eliminating barriers. I am also mindful of my own boundaries as a clinician and a business and have a limited amount of sliding scale or pro bono spots. When those who can use their insurance or pay the self-pay rate it allows for others to utilize this. I believe it balances out eventually.


Roadmap- What to expect

  1. 15 minute phone consultation to find which assessment is the right fit

  2. Informed consent and paperwork completion

  3. Interview/Assessment is completed

  4. Scoring, report, or letter writing is completed by clinician

  5. Feedback session to provide information, education, referrals, and resources
